Hey, Parent, it's great you're here!

I welcome learning about your swimmer, so have a few questions for you (below). A parent's input helps in my planning of the session, gives me a small sense about the person before I work with them and can be beneficial in establishing a rapport, etc (PS - I won't throw you under the bus by divulging anything shared!).

A few weeks prior to the session date, I'll provide your Swimmer with a similar (shorter!) questionnaire. It's not in-depth, yet provides them with the opportunity to think about the situation, to understand that this is a collaborative process and to know that their ideas+input are welcome.

'She won't be able to see across the pool without her glasses, so don't plan on communicating via hand signals', or, 'Harold is shy, but has a wicked sense of humor', or 'Marge is self-conscious about her skinny ankles'. You get the idea...
